This educational activity is designed for physicians, pharmacists, nurses and dietitians who manage clinical nutrition.

Despite continued focus on nutrition in clinical settings, up to one-half of hospitalized patients meet criteria for malnutrition, with higher rates in patients aged 65 years and older. Loss of lean body mass is accelerated during critical illness, and optimal nutrition is an integral factor in recovery from wounds and critical illnesses. Malnutrition in critically ill patients is associated with increased morbidity, mortality, and costs. Yet, there is a significant lack of focus on nutrition therapy by clinicians. Further, nutrition is not considered a vital therapy in critical care settings. Lipid emulsions (LEs) are an integral part of parenteral nutrition. A variety of formulations are available as an energy source alternative to glucose and to prevent fatty acid deficiencies. A number of alternative LEs have been investigated, including soybean, fish oil, and olive oil formulations. Therefore, members of the nutrition support team must be familiar with available and novel LE formulations to differentiate between their biochemical and physiological properties.

Upon completion of this activity, participants will be able to:
• Describe the historical use of lipid emulsions in parenteral nutrition and the rationale for development of alternative formulations
• Discuss the impact of differences in fatty acid composition on inflammation and immune function in critically ill patients
• Critically evaluate current efficacy and safety data of available lipid emulsions across diverse patient populations including critically ill and home parenteral nutrition patients
• Incorporate lipid emulsions into parenteral nutrition, accounting for differences in fatty acids, dosing requirements, and optimal duration of use
