Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is a rare, heterogeneous syndrome that poses substantial diagnostic and treatment challenges for neurologists. While proximal muscle weakness serves as a hallmark of CIDP, atypical variants often deviate from this classic presentation, mimicking other neuropathies, leading to many patients remaining undiagnosed or misdiagnosed and inappropriately treated. For those patients with diagnosed CIDP, while first-line therapies such as intravenous immunoglobulin (IVIg), corticosteroids, and plasma exchange remain cornerstones of management, clinicians need to navigate their relative benefits and risks and be prepared to manage patients with suboptimal responses or experiencing intolerable AEs, including individualized and optimal introduction of novel agents.
